We will contact you when one of our physicians has capacity to accept additional patients.

Attend your first appointment
Bring your health card, medication list and relevant medical information so the physician can review your healthcare needs.

Bring to your first visit
Come prepared
- Valid Ontario health card (or other provincial card)
- Photo identification
- A list of your current medications and doses
- Names of any specialists currently involved in your care
- Recent test results or records from a previous family doctor, if you have them
